This is a very sad beginning, but very well written. I'm not sure I understand why Bill said 'Bil' when he opened his eyes, unless that was a left over remnant from an earlier version where Tom was the one who escaped?...and the anorexia/bulimia might've been a little abruptly introduced, but I think I'm going to like this.
