My guess is that Tom said that he faked it all, thinking that it'd be easier that way for Bill to get over him, but Tom really does love him.
Bill will get to the hospital on time and pull through, and in the end, Bill and Tom will end up together after all.

Okay, I may guess I'll be the only one who likes this chapter -my favourite so far. I've always liked drama and angst and this is just good for that. I found especially evil that Tom lied during pretty much the whole story but well, some people are just assholes, right?
