If bill and tom were fourth cousins twice removed, that would mean that they shared great great great grandparents, and that tom was two generations ahead of bill. This could be possible for a 17 and 19 year old, if Bill's line tended to have kids at about age 20, and Tom's at age 30. (I think I got that right)
